Blended Case Management Program

There are 5 Blended Case Management Teams (BCM), each consisting of an ICM Supervisor an ICM and an SCM. Each team will provide services for 44 high risk/high need individuals, for a total of 220 clients. Priority populations will include:

Þ Assisted Outpatient Treatment (AOT) Referrals

Þ High users of psychiatric inpatient and CPEP services

Þ Jail or prison discharges

Þ Individuals who are homeless

 

The role of the SIBN Case Manager will include the following responsibilities:

Þ Review of the individual’s clinical history, life goals, service needs, and goals for participation in services.

Þ Collaborate with both consumer and provider (s) in the development and implementation of an Individualized Service Plan (ISP) and crisis plan based upon the consumer’s chosen goals.

Þ Monitor both the utilization and efficacy of the service package on an ongoing basis and actively work with both the consumer and provider in revising the ISP when indicated.

Þ Assure ongoing assessments of client goals, strengths, and skill deficits.

Þ Determine resources that will meet the need identified in the assessment process.

Þ Coordinate and integrate a written service plan in cooperation with clients.

Þ Facilitate service delivery.

Þ Coordinate treatment plans with service providers.

Þ Assure the maintenance of case management records.

Þ Provide services that prevent or resolve crisis in order to prevent unnecessary use of emergency rooms and inpatient services.

Þ Provide medication education.

Þ Assist clients in learning to use fiscal resources.

Þ Provide community support and education.
